Job Description:
The John Howard Society of PEI is currently seeking a passionate and motivated caseworker to join our team. As a caseworker, you will play a vital role in providing services and case management to individuals and families who are experiencing homelessness or who are at risk of homelessness. Key Duties/ Responsibilities:
- Report directly to the Wrap Around Housing Program Manager
- Accept referrals from a range of sources including self-referrals, community-based organizations and Government organizations
- Assess client needs based on bio/psycho/social requirements using the Service Prioritization Decision Assistance Tool and internal assessment tools
- Create a goal-oriented case plan for clients which reflects the needs and wishes of the client
- Assist in finding and maintaining safe and secure housing
- Provide a strengths-based, trauma-informed approach which promotes life stabilization when supporting clients
- Clarify processes and procedures of systems so as to ensure clients understand their position.
- Work collaboratively with various community agencies and Government departments
- Organize and participate in case conferences with community agencies
- Connect individuals to resources in their communities to address their housing needs and enhance their support networks. Advocate professionally for clients when required
- Record work undertaken using the HIFIS database
- Record Program data and write reports when required
- Be prepared to work in a range of settings including the client’s home, community settings, health facilities and the Provincial Correctional Centre
- Other duties may be required
Key Skills Required:
- A working knowledge of housing issues and homelessness
- A working knowledge of Federal and Provincial Social Programs
- The ability to work independently as well as in a team environment
- The ability to organize, prioritize and multi-task
- The ability to conduct assessments with clients
- The ability to evaluate risk
- The ability to problem-solve creatively
- The ability to work well under pressure while maintaining a positive attitude
- Strong communication skills, both written and oral and the ability to communicate at a variety of levels
- Have a solid knowledge of and use with tools used to perform tasks (computer, cellphone, printer, scanner etc)
- Possess a good knowledge of the Island’s geography
- Possess a working knowledge of community resources and how to access them
- Demonstrable initiative and drive to seek resources and implement change
- The ability to represent/promote the organization and its values in a positive manner within the community and with clients
- The ability to be a public advocate for your client
- Understand and practice professional boundaries
Qualifications:
- University degree in human services and/or post-secondary education and certification in the field of human services or an equivalent combination of education and experience.
- At least 2 years experience working in client-facing services (in health or social work)
- Experience working with marginalized populations (including but not limited to individuals experiencing homelessness, individuals who use substances, and individuals with mental health issues)
- ASSIST training would be an asset
- CPR- First aid certified would be preferred
- A criminal record check is required
- Must have a valid Prince Edward Island driver’s license and have access to reliable, insured transportation.
- Must be legally eligible to work in Canada.
