Job Description:
Position Summary:
The Day Program Community Support Worker provides individualized support to participants in community-based day programs, helping them develop life skills, increase community integration, and promote independence. The role includes assisting participants in engaging in a variety of structured activities, supporting them in achieving personal goals, and fostering an inclusive, respectful, and supportive environment.
Key Responsibilities:
- Provide one-on-one and group support to participants, focusing on their individual needs, goals, and preferences.
Assist participants in participating in recreational, educational, and social activities that promote skill development and socialization. - Support participants with personal care needs (e.g., mobility, hygiene, eating, dressing) when necessary.
Facilitate community outings and engage with local services and businesses to encourage community participation.
Work with the interdisciplinary team to develop and implement person-centered plans and goals for each participant. - Monitor and document the progress of participants in achieving their goals, noting any changes or developments.
Ensure the safety and well-being of participants, providing guidance and support in a crisis or emergency situation. - Promote positive behaviors and teach coping strategies to help participants manage challenges.
- Communicate effectively with families, caregivers, and other professionals involved in the participant’s care.
- Maintain a clean and organized program environment, ensuring materials and equipment are safe and accessible.
- Comply with all organizational policies, procedures, and regulatory requirements.
Qualifications:
- High school diploma or equivalent (required). A post-secondary degree or certification in social work, psychology, education, or a related field is preferred.
- Previous experience in community support, social services, or a similar role working with individuals with disabilities or other support needs.
- Strong communication and interpersonal skills, with the ability to interact positively with participants, families, and team members.
- Knowledge of person-centered planning, support strategies, and behavioral management techniques.
Ability to handle challenging situations calmly and professionally. - First Aid and CPR certification (or willingness to obtain).
- Valid driver’s license and working vehicle( 2 million liability insurance ).
