Job Description:

Background:

United Way of PEI works to break down barriers, improve lives, and create opportunities for all residents to thrive. Our signature service, 211 PEI, embodies this by providing a 24/7/365 live answer navigation service and searchable website that is a gateway to community, social, non-emergency health, and government services. 211 services are a trusted source across Canada and the United States. As such, the service will play an important role in the new health care model being rolled out in PEI.

Health PEI in partnership with the Government of PEI’s Department of Health &; Wellness, are creating Patient Medical Homes across the Island. Providing primary care, these homes are a shift from solo family practices to collaborative health care and better access for Islanders. The homes lay at the centre of the model as the physical locations where Islanders receive care from a variety of health care professionals, including physicians, nurse practitioners, registered nurses, social workers, and dietitians among others. Each home will exist within a larger Medical Neighbourhood, which may include multiple homes. 

This position will embed the 211 service into medical homes and neighbourhoods to support community members in navigating community / government social services.   

Overview of the Position:  

Through education and outreach, the 211 PEI Educator will be responsible for ensuring that the 211 PEI service is a key, go-to-support for staff and patients within Medical Neighborhoods and the surrounding communities.

This will entail:

– Training Educating clinical and non-clinical staff in the province about the 211 PEI service.
– Increasing awareness of 211 PEI within other health care services located within Medical Neighborhoods (e.g. Health Care Centers).
– Providing in person outreach activities to the public.
– Creating a structure where 211 can be the backbone for service providers and integrated within Health PEI and the Department of Health and Wellness onboarding and regular training and education opportunities.  

The above will take place through relationship building, one-on-one and group in-person and online presentations, and outreach activities.

Key Duties and Responsibilities:

– Develop and implement an education strategy for staff and clients around 211 PEI.
– Develop engaging and informative 211 PEI presentations and webinars.
– Work with all United Way of PEI’s staff to develop new and future promotional materials to support education around 211 PEI.
– Perform outreach through in person and online mediums.
– Ensure that outreach and education comply with Inform Canada/Inform USA (formerly Alliance of Information and Referral Systems (AIRS)) certification standards.
– Other duties assigned to support the outreach efforts of 211 PEI.
– Other duties assigned to support United Way PEI

Education and Experience: 

– Certificate or diploma in Communications/Public Relations, Education, related social or health field.
– Recent related experience of five years plus; or an equivalent combination of education, training, and experience; or other qualifications determined to be reasonable and relevant to the level of work.
– Strong knowledge and understanding of health care and the community services sectors in PEI. 

Assets: 

The following attributes will be considered as assets in performing the duties of this position: 

– Demonstrated interest or experience in the non-profit or volunteer community service sector.
– Experience working within health care system.
– Bilingualism (French and English) in both the oral and written language.
– Comfortable using software tools to design creative webinars and outreach materials.

– Capability to develop and deliver presentations to diverse groups and health care workers in an engaging and effective manner.<

– Must possess a valid driver’s license and vehicle.

Position Type/Expected Hours of Work:

This is a full-time position, Monday-Friday 8am-4pm (35 hrs/wk). However, some flexibility may be possible, and flexibility is required for some after-hours work, which is compensated as per United Way of PEI’s policies. This role will be a hybrid of working from the Charlottetown office, home office and travel across the province.   

Travel:

Travel throughout PEI is required for this job. Where possible, travel will be during the business day. Some overnight travel will be required. Out-of-province travel is required during the training period for this role.
