The Mount Continuing Care Community is currently seeking LPN’s for our long-term care facility in Charlottetown, PEI. This position reports to the Registered Nurse Director of Nursing, and assumes authority, responsibility, and accountability for the delivery of nursing services in our continuing care community.
Purpose of Position:
- Licensed practical nurses (LPN’s) provide nursing care and administer medications under the supervision of the registered nurse.
- They work collaboratively with other members of the interdisciplinary team and have a supervisory role with RCW staff.
- They are responsible and accountable for their actions and practicing within their scope of practice, the LPN code of ethics, LTC Clinical Standards, nursing department policies and procedures and the mission, vision, values and Person Centered Care Philosophy of The Mount Continuing Care Community.
- When the LPN realizes that the resident’s needs are beyond the LPN scope, they notify the RN to assess.
The successful candidate will possess the following qualifications: (please note only candidates with the following qualifications will be considered)
- Completion of a university or college, or other recognized registered nursing program.
- Current registration with CLPNPEI.
- Ability to work as a team member and ability to meet the physical requirements of the position.
- A valid criminal record check and vulnerable sector check.
- Up to date CPR training.
- Up to date immunizations and TB testing.
Main Responsibilities include but not limited to:
- Demonstrate professionalism in all actions
- Demonstrate leadership
- Demonstrates safe and effective use of devices and equipment
- Communicates and works effectively with all residents and family members as well as other team members and supervisors
- Participates in resident move ins, discharges and transfers
- Perform assessments, conduct planning, implement plan of care and assess effectiveness of plan of care on an ongoing basis.
- Participates in physician rounds and processed physician orders
- Prepares and administers medications by various routes
- Monitors, reports, and records resident’s response to medication administration
- Provide nursing services, within defined scope of practice, to patients based on patient assessment and care planning procedures.
- Answer call signals; assist with elimination needs; assist with resident hygiene, dress and groom residents; serve meal trays, assist in feeding of residents and assist residents with menu selection; weigh, lift, turn, and position residents; assist with ambulation and mobility; set up and provide leisure activities for patients, accompany patients on outside recreational activities and perform other duties related to patient care and comfort.
- Perform nursing interventions such as taking vital signs, applying aseptic techniques including sterile dressing, ensuring infection control, monitoring nutritional intake and conducting specimen collection.
- Specialized care including catheter care, colostomy care, skin and wound care, monitoring of oxygen equipment
- Assess, monitor and provide for resident comfort and pain relief
- Monitor patients’ progress, evaluate effectiveness of nursing interventions and consult with appropriate members of healthcare team.
- Provide safety and health education to individuals and their families.
- Administer, record and monitor medications and treatments as prescribed by a physician or other practitioner.
- Transfer, lift, and reposition residents.
- Monitor the ongoing physical, social, emotional and spiritual needs of each resident.
- Monitor, assess, address, document and report symptoms and changes in residents’ physical, emotional, social, psychological and spiritual conditions
- Performs basic first aid measures and emergency procedures including CPR and Heimlich maneuver
- Provides for the needs and wishes and necessary support of the resident and family in end of life care
- Ability to handle sensitive information with the highest degree of integrity and confidentiality.
- Participate in resident care conferences and assists with the revision of goals, priorities and nursing interventions
- Other duties as required.
