Job Description:

The Community Health Nurse (CHN) will deliver a comprehensive community health nursing program that addresses health concerns identified by Abegweit First Nation.
The CHN will have the knowledge of concepts, principles, practices and theories of community health nursing, population health, disease and injury prevention, and health promotion. The comprehensive community health nursing program may include but not limited to; communicable disease control, maternal child health, chronic disease prevention and management, individual/group health counseling, immunization programs, and school health.
The CHN will collaborate with other members of the wellness center’s interdisciplinary team in the delivery of services.
Other related duties as identified in response to ongoing community health needs.
