Job Description:
Inclusions East requires a Transition Employment Worker on a contract pending renewal. Position will require travel within Kings County.
A Transition Employment Worker will narrow the gap between the school/training centre and employment in the community. By having this position; it will aid in building the relationship that enables individuals seeking employment to have the greater chance for success.
Transition Employment Worker (Transition may be school-to-work or day program-to-work)
Duties include but are not limited to:
- To consult with significant others and develop a transitional plan for clients in co-ordination with Senior Staff and Employment Practitioner. Plans will focus on needs for employment and inclusion in the community.
- To meet and assess employment needs and barriers of clients whether on-site or outreach
- To secure community placements/involvement with Employers by and providing follow-up co-ordination, monitoring and support. This includes providing job coaching, client scheduling, etc.
